**To avoid damage to the equipment, locally we do not deliver in the rain. All deliveries will be rescheduled to next available sunny day.
* We provide you with a 2 hr window starting at 8:00 - 10:00 a.m.
Delivery charges are based on: M-F 8:00-3:00, 1 man delivery, Service elevator, No special building requirements or permits, No holidays.
There is an additional charge if the building requires a certification of insurance

Delivery Terms & Conditions:
Pick up and Delivery charges apply if the equipment is picked up by our company.
There is extra charge for Palletizing and Crating. (Palletizing: $200.00, Crating: $500.00)
Parking Charges: Parking must be paid by the customer.
The pickup & delivery pricing is based on 1 load, 1 trip and 1 person delivery, Service Elevator & Regular Hours .
1 Load: Approximately 1/2 of a pick up truck.
Heavy Items: There is additional fee required for this delivery. Such as: Surgery Tables, Hospital /Surgery Center Blanket Warmers, Large Sterilizers, Procedure Chairs, Stretchers, etc.
2 people delivery: There is additional fee required for this delivery. Some of the odd shaped equipment also may require additional delivery person.
Service Elevator: There is additional fee required for this delivery if the facility does not have a service delivery elevator. The facility should have easy access for the delivery of the equipment. (i.e. Service elevator vs. stairs)
Regular Hours: Weekdays between 7:30 - 3:30 p.m.

All repair are conducted at our facility, you can drop off the equipment and/or request pickup and delivery.
You are responsible for pickup and delivery charges if applicable.
We must have a completed and signed credit card authorization form on file for all repairs. However, your credit card is not charged until your repair is completed. If you wish us not to use the credit card on file for payment, please coordinate with the office ahead of time.
There is an initial $90.00 diagnosis fee which is due before we start.
There is a minimum of 1 hour labor applied to all repairs.
Service Labor Rate:
Medical Equipment: $105.00/Hour
Optical Equipment: $125.00/Hour
We will notify you with the total cost of the repair. You must provide your approval is required for all repairs before we can proceed with the repair.
If you choose to proceed with the repairs, the initial diagnosis fee will be waved.
If you choose not to proceed with the repairs, your repair is considered completed and you will only be charged for the diagnosis fee of $90.00 plus the pickup and delivery cost when applicable.

We provide free loaners to our customers as a courtesy. Please note, you are responsible for any damages to our loaners.
Free Loaner Policy: Due to our limited number of loaners, the following terms and conditions apply.
- You will be provided with a free loaner if available
- You must sign a free loaner agreement & leave a check or valid credit card information as security deposit. (We must have a completed and signed credit card authorization form if using credit card)
- Once you are notified of the repair costs, you must contact us within 3 business days with your decision to proceed with repairs or not.
- If you decide to proceed with the repairs, you can keep the loaner while the repairs are being made. You will be contacted once the repairs are completed.
- If you decide not to proceed with the repairs, the repair is considered complete and the loaner must be returned within 3 business days and/or it will be converted to rental and a rental fee will be rendered.

We require a completed and signed credit card authorization form for our records prior to scheduling calibrations. The credit card information is needed as a form of security deposit only.
We provide calibration services in house (Our facility) and/or on site (Your facility)
There are different prices for in house and on site calibrations.
There is a service call fee for calibrations at your facility.
Service call fee is based on the distance and is the same as our delivery fee.
For onsite calibrations we require a list of all the equipment which requires calibration, this will assist us with scheduling the technician's time as well as bringing the right tools.
CHDP Approved: Our technicians are certified by FDA Division of measurement and weight devices

Warranty:
Our equipment can only be serviced, repaired by Angelus Medical & Optical Equipment and/or our representative. The warranty is void, if the equipment is serviced, repaired, tampered with by another person/company.
Angelus Medical & Optical Equipment is not responsible for damages occurred during shipping if delivery was not completed by Angelus Medical & Optical Equipment or their representatives.
Our warranty does not cover mishandling and abuse of the equipment.

Unless otherwise specified, we provide white glove delivery through our shipping partners.
White glove is to deliver items to your office, unwrapping them and removing the debris.
All shipments are insured and must be inspected by the recipient prior to signing the delivery document.
Please take pictures and document any possible damages during the shipping if any.
MAKE SURE TO INSPECT THE ITEMS PROPERLY BEFORE YOU SIGN THE SHIPPING DOCUMENTS.
Please note, your signature and approval of the delivery makes you responsible for the damages if any and we will no longer be able to hold the shipping company responsible.
Returns and exchanges:
Refurbished/used Items can be returned within the first 10 days from the purchase date.
New items can not be returned once the order is placed with the manufacturer.
Taxes and shipping cost is non-refundable.
All customized items are non-refundable.
There is a 20% restocking fee applied to all returned items. NO EXCEPTIONS.
Items can not be returned after 10 days for refund. NO EXCEPTIONS.
ALL ITEMS RETURNED AFTER PURCHASE, we reserve the right to offer the customer store credit and/or a similar item of the comparable value.
All customized items are non refundable and non exchangeable.
